NTSB: Spatial Disorientation, Radar Fault Caused Wigwe’s Fatal Air Tragedy

According to the report, the tragic crash—which occurred on February 9, 2024, near the California-Nevada border—was primarily caused by the pilot’s decision to continue flying under Visual Flight Rules (VFR) despite deteriorating weather conditions that required Instrument Meteorological Conditions (IMC). The pilot reportedly became spatially disoriented, leading to a loss of control and collision with terrain.
The investigation further uncovered serious safety management failures on the part of the helicopter company. These included neglecting to complete flight risk assessments, failing to log maintenance issues, and not ensuring compliance with Part 135 federal aviation regulations before takeoff.
Notably, the pilot had raised concerns via text message about a malfunctioning radar altimeter during an earlier leg of the trip. Despite being aware that the radar system remained faulty, the flight still proceeded to pick up passengers. The company mechanic attempted but failed to fix the altimeter before departure.
Additionally, there was no recorded discussion between the pilot and the flight operations team regarding the faulty equipment or adverse weather before the fatal flight. The NTSB concluded that this chain of oversights—combined with the pilot’s critical decision-making error—led to the deadly crash.
The tragedy continues to draw attention in Nigeria, where the late Wigwe’s contributions to banking, business, and philanthropy left a lasting legacy.
