During a press conference, Kenton County Commonwealth Attorney Rob Sanders emphasized the avoidable nature of the incident. “This shooting death was caused by the fact that two adults left a loaded handgun with a round in the chamber within reach of a three-year-old child they did not supervise,” Sanders declared.
Farrell, the mother, has been arraigned on charges of second-degree manslaughter and other offenses, as per court records. Meanwhile, Adams, the father, is currently held on second-degree manslaughter charges and is yet to be arraigned.
Police reports indicate that the 2-year-old boy was fatally shot in the family’s apartment, leading to his immediate transport to the hospital, where he tragically succumbed to his injuries.
Adding a layer of complexity to the case, it was reported that Farrell allegedly fled the scene before law enforcement arrived and did not appear at the hospital where her son, Khalil Adams, passed away. Sanders revealed that Farrell justified her actions by expressing a fear of being held in jail and potentially missing her child’s funeral, despite fleeing while the child was still alive.
The family’s living situation was unveiled during the investigation, depicting a cramped one-bedroom apartment where the loaded handgun was kept “for protection.” Sanders disclosed that the parents and their children shared the living space with another person, with the family sleeping on a floor mattress while the toddlers rested on a couch.
U.S. Marshals successfully located Farrell, who was found hiding in a hotel room in Florence, Kentucky, and apprehended her on an outstanding probation warrant related to a prior felony conviction. The arrest also included Adams and another individual with no apparent familial relationship to the parents.
Sanders noted that Farrell had legally purchased the gun from a federally licensed arms dealer. While the surviving 3-year-old toddler sustained no physical injuries, the tragedy has left a community in mourning, and the parents now face legal consequences for their alleged negligence.
As the legal proceedings unfold, the incident serves as a stark reminder of the critical importance of responsible firearm ownership and child supervision to prevent such devastating accidents.

The incident occurred on March 23, 2023, when Halilu and an armed gang abducted Offo from his hotel, the Everready Hotel. Although Offo’s family paid a ₦2.5 million ransom, he was killed after recognizing Halilu—his own neighbour—among his abductors. Halilu was eventually arrested by the Department of State Services (DSS) on December 4, 2025, and was also linked to the April 2, 2023, kidnapping of Ibrahim Abdullahi in Lokoja, for which a ₦5 million ransom was paid.
